Dental Management of Patients with Sulfonamide (sulfa) antibiotic allergy

Avoid sulfonamide antibiotics (allergy)
Record of sulfonamide antibiotic allergy. The points below cover relevant oral side-effects and drug interactions so prescribing and treatment remain safe and non-conflicting.
Emergency vs routine management
If EMERGENCY — what to do
- • Manage emergencies normally with standard precautions.
If ROUTINE — what to do
- • Proceed with routine care — standard dental precautions apply.
Clinical precautions
- • Avoid sulfonamide antibiotics (co-trimoxazole). Standard dental antibiotics (amoxicillin, azithromycin, doxycycline, clindamycin) contain no sulfonamide group.
- • Local anaesthetics, including articaine, are not sulfonamides — they can be used normally.
- • Do not confuse with SULFITE allergy: if the reaction was to sulfites (preservative in adrenaline-containing cartridges), use plain mepivacaine 3% or prilocaine 3% with felypressin.
- • Cross-reactivity with non-antibiotic sulfonamides (furosemide, thiazides) is very low.
- • Avoid prescribing drugs that interact; verify any new prescription against the patient's full medication list.
Medications to avoid → safer alternative
