Dental Management of Patients with NSAID / aspirin allergy or sensitivity
Important — second opinion only. DentiCision provides an evidence-based second opinion to assist the dentist. It does not replace the treating dentist's professional clinical judgment or consultation with the patient's physician. The treating dentist retains full clinical, scientific and legal responsibility. Always verify drugs and doses against current guidelines.
Avoid NSAIDs & aspirin (hypersensitivity)
Record of NSAID / aspirin hypersensitivity. The points below cover relevant oral side-effects and drug interactions so prescribing and treatment remain safe and non-conflicting.
Emergency vs routine management
If EMERGENCY — what to do
- • Do not delay emergency treatment — proceed with urgent care while applying the modifications listed above.
If ROUTINE — what to do
- • Proceed with routine care once the listed precautions, labs or timing checks are satisfied.
- • Book at the most suitable time and keep appointments short where advised.
Clinical precautions
- • Record the reaction: respiratory (NSAID-exacerbated asthma/rhinitis), urticaria/angioedema or anaphylaxis.
- • Use paracetamol for pain (single doses up to 1 g are tolerated by most NSAID-sensitive patients).
- • Medical emergency: do NOT give aspirin for suspected myocardial infarction — call emergency services and follow the emergency team's advice.
- • Selective COX-2 inhibitors (e.g. celecoxib) are usually tolerated but only after allergist assessment — not as first choice in practice.
- • Avoid prescribing drugs that interact; verify any new prescription against the patient's full medication list.
Medications to avoid → safer alternative
- NSAIDs & aspirin — ibuprofen, diclofenac, naproxen, mefenamic acid, ketorolac, aspirinParacetamol (max 4 g/day)
Documented NSAID/aspirin hypersensitivity — cross-reactivity across COX-1 inhibitors.
