Dental Considerations for Patients Taking Ustekinumab (anti-IL-12/23)

Patient on ustekinumab. Invasive care can usually proceed with infection vigilance, correct drug timing, and rheumatology coordination for major surgery.

If EMERGENCY — what to do

If ROUTINE — what to do

Clinical precautions

  • • Meticulous infection control; treat oral infection promptly
  • • Biologic immunomodulator — infection vigilance; coordinate with the prescriber for major surgery.
  • • Plan invasive procedures at the drug trough; discuss withholding for major surgery with rheumatology
  • • Antibiotic prophylaxis is NOT routinely required for immunosuppression alone — decide case-by-case with the treating physician for invasive/surgical procedures. If indicated: Amoxicillin 2 g orally 1 hour before (adult); if penicillin-allergic, Clindamycin 600 mg or Azithromycin/Clarithromycin 500 mg orally 1 hour before.

Information & tests to request

  • • Absolute neutrophil count — ANC (x10⁹/L) — ustekinumab raises infection risk and can lower blood counts. Check an FBC; proceed with invasive care when ANC ≥1.0 (ideally ≥2.0).
  • • Days since last biologic dose — Plan invasive surgery at the drug trough (just before the next dose). For major surgery, coordinate with rheumatology about withholding a dose to reduce infection risk.
  • • Daily prednisolone-equivalent dose (mg) — This patient's condition/therapy is commonly associated with long-term corticosteroids, which can suppress the adrenal response to surgical stress. Enter the daily dose (0 if none) to determine whether supplementary stress-dose cover is needed.
