Dental Management of Patients with Multiple sclerosis (MS)

Important — second opinion only. DentiCision provides an evidence-based second opinion to assist the dentist. It does not replace the treating dentist's professional clinical judgment or consultation with the patient's physician. The treating dentist retains full clinical, scientific and legal responsibility. Always verify drugs and doses against current guidelines.

Multiple sclerosis — proceed with precautions

MS itself does not contraindicate dental care. Risks come from fatigue, dysphagia, trigeminal neuralgia and immunosuppressive disease-modifying therapies (ocrelizumab, ofatumumab, natalizumab, fingolimod, cladribine, alemtuzumab).

Emergency vs routine management

If EMERGENCY — what to do

  • • Do not delay emergency treatment — proceed with urgent care while applying the modifications listed above.Ref: National Multiple Sclerosis Society — Disease-modifying therapies for MS (clinical resource)

If ROUTINE — what to do

  • • Proceed with routine care once the listed precautions, labs or timing checks are satisfied.Ref: National Multiple Sclerosis Society — Disease-modifying therapies for MS (clinical resource)
  • • Book at the most suitable time and keep appointments short where advised.Ref: National Multiple Sclerosis Society — Disease-modifying therapies for MS (clinical resource)

Clinical precautions

  • • Short morning appointments (fatigue and heat sensitivity); good chair support and breaks
  • • On immunosuppressive DMTs or recent IV steroid pulse: check a recent FBC (lymphocytes / ANC) before surgery and review promptly for infection
  • • Also add corticosteroids or other immunosuppressants to the evaluation if taken
  • • Trigeminal neuralgia or facial numbness may be an MS symptom — refer new neurological signs to the neurologist
  • • Dysphagia: semi-upright position and high-volume suction
