Dental Management of Patients with Dental anxiety / nervous patient (fainting risk)

Anxious patient — stress reduction & syncope readiness

For a nervous/anxious patient use a stress-reduction protocol to prevent a vasovagal faint (the commonest dental emergency). If vasovagal syncope occurs (pallor, sweating, slow pulse, loss of consciousness): stop treatment, lay the patient flat and raise the legs, ensure airway, give oxygen, and they usually recover quickly.

If EMERGENCY — what to do

  • • Vasovagal syncope: stop, lay flat with legs raised, loosen tight clothing, oxygen 15 L/min, monitor pulse; if not recovering within 1–2 min, treat as another emergency.Ref: Resuscitation Council UK. Medical emergencies and resuscitation — Standards for dental practice, 2020 (updated 2023)

If ROUTINE — what to do

  • • Consider inhalation (N₂O) or IV midazolam sedation by trained staff when needed.Ref: SDCEP. Conscious Sedation in Dentistry, 3rd ed., 2017

Clinical precautions

  • • Short, morning appointments; calm, reassuring manner; explain each step
  • • Effective local anaesthesia; consider topical first
  • • Vasovagal faint: lay flat + raise legs, airway, oxygen; monitor recovery
  • • Consider referral for conscious sedation if anxiety is severe or prevents care
